I would say it’s probably

Academics/school tier > Research >>>> Advocacy >>> interest group leadership

Yeah I have research, advocacy work and ig leadership as well. But I felt like it would be glaringly obvious to them that I’m picking the easiest role to just say I had a leadership position. I go to a mid tier tho so all of this may be useless lol

I doubt mid tier will hurt unless you’re trying for neurosurgery at Hopkins or something even then plenty of mid tiers on the roster. I think going to a low tier hurts more than going to a top tier helps. Mid tier is fine

The mid tier nature of your school is sort of beside the point, but regardless having any officer position in an IG is unlikely to be helpful in and of itself. The real point of taking a leadership role in these organizations is that it helps you network with mentors in your field, and in some cases may help you make connections to start research. If it's just a line on your CV that you had an officer position that isn't going to help you regardless of whether you were secretary, president, or grand poobah.
 
If you are doing them exclusively for your CV, the return on investment for the time you spend on extracurriculars is not worth it. An extracurricular is a worthwhile investment (for your career or you personally) if you'd still want to go even if you couldn't put it on your CV.
