I’d also check each of the schools to make sure that they have home residencies for derm and rads.

Are you hoping to match in the East coast? I've heard that some students applying to CA programs got shut-outed because they were no longer state residents, after attending oos med schools. Personally, I would stick to the school closest to home. The climate differences alone would be a miserable experience. Also, I'm not sure where you're getting the idea that Einstein is high prestige. The last I've heard of them, they were being dragged for forcing their 4th year students to "volunteer" on the front lines with no PPE during the height of COVID as a graduation requirement.

The issue is I'm not sure where I want to match, no big preference for east or west coast. For the ranking thing I just saw that Einstein is ranked 35 or so on USN while Irvine and Georgetown are around 50

Maybe on a national scale Einstein might be considered a well ranked school, but you have to remember that you would then be judged in comparison with all the other NYC schools: NYU, Columbia, Cornell, Hofstra etc.. that are less than a 5 mile radius from Einstein when it comes to applying to residency. Once you're actually in NY/ the east coast, the name Einstein will not really be considered prestigious. I would stick to CA, especially if I wanted to match into a competitive specialty. It's pretty rough out here on the east coast. There's a Youtuber who attended Yale who failed to match derm this year even after taking 2 research years.
