I'm a incoming pain resident with a neurology background and will start looking at jobs this summer/fall

1) do you recommend a HOPD or PP as your first job?

2) what things to watch out for or look out for when negotiating contracts

3) besides SDN, what are other resources I can use to look for jobs?

4) i want to live in a midsize city in the south or east but I'll be willing to travel 30-40 minutes for a more rural job. How much of a difference would it make on terms of compensation?

5) speaking on compensation, what's a good salary for 1st year attending in the south or east? (Not big cities but more rural?)
 
I recommend HOPD as it usually offers higher guaranteed salary with better benefits in more secured setting. PP if you plan to open your own shop in the near future. One thing you should know is that once you work for the hospital for a while in a busy setting and you make enough salary with easier schedule, it is hard to switch to PP or open your own shop cause you have to be ready for a pay cut for a couple of years or longer. I mean it could be half or even less in the beginning.

if you have any inkling that PP may be for you or you cannot stand hospital administrators then start off in PP, you can always transition to HOPD
2) what things to watch out for or look out for when negotiating contracts
sign on bonuses and moving expenses are important but not nearly as important as the salary you start with and the bonus structure. your $/wRVU is all that matters in HOPD so calculate that out at multiple wRVU thresholds and make sure it doesn't decrease the more you produce. try to avoid a non-compete or make it very small and a year at most. make sure the practice/hospital pays tail insurance if you leave.

all the specialty societies have job boards, www.gaswork.com (click the always box for pain management), contact practices directly that you're interested in to see if they have a need

very location dependent

PP probably starts 250-400 these days, HOPD may start you closer to the MGMA median
 
Whats the highest salary have you guys seen starting out of fellowship? There's a pain doc on reddit (I think he's 2 years out of fellowship) and said his base 750k with productivity bonuses which brings it close to 950k. This was all starting year one. I believe he said it was a HOPD position in a rural area about 40 minutes from a midsized city. Does that sound realistic? Has anyone seen offers like that?

There’s me, who would be happy with 350 to 450k base salary after fellowship.

Well he claims he's doing 4 days/40 hours a week.
