Been a long time reader, first time poster...
So I did an anesthesia rotation, got a letter from my clerkship director (whom I worked with very closely and she offered to write a letter). Then as per the advice of everyone and their mom, I asked the Chairman for a letter (worked with him on 3 cases, he said "yes, of course"). He's from the same program.

So my question: should I send both letters to my programs? if only one, then which one would you recommend? Has anyone done that? I feel like the clerkship director letter is a strong one while the other can be the more weighted letter. My third letter is a strong Internal Medicine letter, also have a Neurology letter just incase need a 4th.

definitely the clerkship director letter.the chairman letter is definitely a maybe-- if those three cases you worked with them you feel they really got to see you in action and you had meaningful conversation with them go for it. If you have any inkling that it will be generic, go with the three strong letters. Good luck.
