These have been on my mind the past 24 hrs.

1) What is the difference in presentation between endometriosis and leiomyoma? They both just seem like pain/bleeding that gets worse with your cycle.

2) What is the most common organism found in endometritis? UWorld claims it is Bacteroides but Goljan RR claims GBS. Which is it?
 
I would like some clarification on 1 also. I can't quite seem to distinguish between them properly.

1. There is very little difference in clinical presentation. A bimanual examination may identify larger fibroids and there may be pain with intercourse. Fibroids are also associated with early termination of pregnancies while endometriosis is associated with ectopic pregnancies in addition to early termination. If you're worried about fibroids, just do an ultrasound to find the sucker.

2. They may both be right. According to UptoDate, 70% of endometritis is polymicrobial and may have both bacteroidies and GBS. According to the sanford manual, you treat endometritis with the assumption that it is polymicrobial.
 
I think leiomyoma you see on an ultrasound or maybe a pelvic exam, endometriosis you don't. I think based on pure physical presentation it would be very difficult to distinguish.

Leiomyoma --> Always pops into my head w/ a black women experiencing menorrhagia.

Endometriosis --> Extreme pain associated w/ onset of menses menses.
 
I think on questions where you have to distinguish between the 3 they'd better give you more info, such as histologic findings or something. I know on one UW question you could narrow it down to endometriosis and leiomyoma, but they showed a histologic picture that would lead you to the right answer.
 
adenomyosis will cause increased uterus size and endometriosis won't.
hmmm i missed a Uw question because i picked adenomyosis...ill need to go back and see if they mentioned anything about uterine size. I bet they did and i just loooked over it.

I think eventually you will see the "opposite" question, where the answer is adenomyosis and the uterus is increased in size...that's where I picked up that little factoid 🙂

1) Pain with defecation (during menses, but not at other times) for endometriosis is pretty characteristic. (Involvement of pouch of Douglas)... I don't think you'd get that with leimoyoma. But someone please correct me, if I'm wrong.

I had a question that said MCC of endometritis post-partum is E. Coli.
