2009 ADA quick question

Hey everyone!

For the 2009 ADA DAT that everyone orders, is it just the form and money order/ check that you mail out, or is there something else?

Also, does anyone know how long it takes to receive? I'm wondering when I should order it so it comes in on time.

You only need the form and a money order. Do not use your personal check. I got mine in 6 days.
