If I'm remembering correctly, the form tells you where to mail it to.

Yeah there's a PDF on google but some areas are kinda blurry and words get cut out. Also 4 problems in the hole punching section are missing. But hey, beats wasting 40 bucks and waiting three weeks to get the thing. Can't believe they charge so much for a test that has so many flaws in the answer key and questions
