If a program decides to give out second looks only to specific candidates, how far up on the rank list are they? I know there's not a clear cut answer, but I just wanted to see what the practice is for those programs who do give out the 2nd looks.

They are, without a doubt, in the top 100% of the rank list. Any other number you are quoted is just a wild-ass guess.
