How big of a setback is this?

One possibly troublesome thing is that my coursework is pretty much all considered science/math, so I'm not well-rounded that way.

We talked about that in your last thread. Go volunteer at a local hospital 5 hours a week in your year off.

Where else should I apply to cover my gap year? I don't know what workplace would allow me to apply to MD/PhD programs and work at the same time.

Try to find a lab anywhere that will take you as a poorly paid assistant for a year. Hopefully there's one at your undergrad or the area that will do this for you. Send out e-mails to profs looking for a position. Talk to your advisors in your major and see if they might know of any positions.
