a few ADSAS questions...

1) when answering the questions about extracurricular activities, volunteer activities, manual dexterity, work experience, and dental experience...how much detail should i go into? should i just list them, or list them and provide a brief explanation for each?
2)Also, if I have hospital shadowing experience from a few years ago before I knew i wanted to go into dentistry, is it worth noting that or should i just note my dental experiences?
3) i feel ******ed asking this, but just to make sure..when it asks for high school rank the 94th percentile range is higher compared to 1-10 correct?
4) On the section where we are to list evaluators...does this mean we list all those that are writing LORS? also, when my advisor sends in my LOR"s , do they each have to a ADSAS matching form attached to them?

1)i think you should list and give brief explanation. to read or not to read is up to them, but you should let them know exactly what you did.
2)if you feel your dental xp is weak, then list the hospital xp too. but, if your dental xp is strong, i don't think there's need for anything else.
3)you're right.
4)correct, list the ppl writing lor's. my school's health prof advisor just needed one....should be same w/ your school.

I am not exactly sure what this question is asking:

Has your education ever been interrupted or affected adversely for reasons other than deficiencies in conduct or academic perfomance?

Is this the place to explain sudden drop in grades due to personal/family issues?

Yes - for example, my mother was ill for a number of years which didn't allow me to leave her to attend d-school in a different state. She passed away, so I'm "back in the game". So, based on my mother's illness and me taking care of her, my education was "interrupted" due to a family situation. Hope this helps.
