Hi, I am not sure where i need to post this so i just decided on this lol.. Ok I am currently taking a Pharmacy tech course at a community college and the teacher is umm I don't know how to say it nicely ..she just doesn't know very much lol. We had a problem in class and she doesn't know the answer to it so she doesn't know how to explain it so i was wondering if anyone on here could help explain it to me(and her lol), ok here is the question.


A Pediatrician ordered ampicillin 50 mg/kg/day IV in equally divided doses every 6 hours for an infant who weighs 26 lbs. Ampicillin 125 mg/5 mL. is available. How many mL of this medication should the baby recieve?

A. 0.5 mL
B. 1.25 mL
C. 1.5 mL
D. 2.0 mL

OK - here's how:

26 lb divided by 2.2kg/lb = 11.8 kg

11.8 kg x 50mg/kg/day = 590mg/day

590 mg/day in 4 doses = 147.7mg

147.7mg (5ml) divided by 125mg = 738.7mgmg/ml divided by 125mg

= 5.9ml

Tell mom to give 6ml (actually a teaspoonful will work too, but I wouldn't put that on the label....but then, I wouldn't put 5.9ml either)

There ya go!

= 5.9ml

Pleeeeeaaaaase!!!!!

This is a mis-written question!

Ampicillin for injection is a dry powder! Even you know that & you haven't mixed an ampicillin for at least as long as I have!

It also has a half life of about 1 hour in sterile water & needs to be put in D5 or NS within that time frame....so its just not sitting around as 125mg/5ml - it comes in a 250mg, 500mg or 1Gm vial. You mix it & take the fraction....

So...I'm going on a mis-written question.

But - I'd still go with 6ml IV po ...whatever!!!!

Yes..I'm working from home....I was in San Antonio yesterday and today...beautiful city..took the family with me...I actually attended a P&T to present.

I just want a GPS.... It's hard to drive in Vermont and Maine..without one...

Yeah, I also get 5.9 mL. Maybe the units on the answers are wrong and they were meant to be in teaspoons (5.9 mL is kinda like 1.25 tsp). BUT, then that makes no sense since it is IV. As sdn1977 said, it's a bad question.
 
the question doesn't make any sense...

perhaps your instructor meant Amoxicillin? it's available as 125mg/5ml powder for suspension.

if that were the case, i have the same answer as the zpak and sdn.
if the kid were in-house i would stick with 5.9. if it was an outpatient, i would have a chat with the prescriber and suggest 1 teaspoon. it will be easier for the parents to measure and a common measure like 5 ml or 1 teaspoon will hopefully make it less likely for them to make a dosing error.

i do it all the time with our peds patients. ie, a 12 year old [pretty much an adult, esp the obese ones] who is ordered 2.5 grams of Timentin, which we have commercially available as 3.1 gm. Why risk making an error while making an odd dose when there will be no harm to the patient with using the pre-mix. We like to use as much pre-mix as possible.

147.7mg*5= 738.5***

Math teacher says rounding errors cannot be forgiven, but screwing up a decimal is even less so. lol😉
