I would leave this formatting at the discretion of your LOR writer. Most LOR writers are familiar of the process. In fact, you might annoy them if you give them too much instruction such as how many paragraphs to write.

1) LOR are usually divide into paragraphs. Depending on the writing style of the person and how much they are willing to write.
2) The info about the writer is usually at the end. When they sign the letter, they might put their contact info below (DameJulie, Professor of XXX department at XX university, Address, phone number, email).
AAMC don't care for the formatting of the LOR. If you have the professor's letterhead and signature, that would satisfy most school's requirement.

If you want to make it easy for your writer, just ask them what material they need to write you the LOR (transcript, etc).
