AACOMAS transcript rquest forms

Does the AACOMAS require to have a transcript request form sent along with official transcripts like the amcas? If so, where is the request form?

Should I just request to send out my official transcripts from my school to the aacomas address? The whole format is new and I am confused.

Under Colleges Attended there is a giant button for each school that says Transcript Request Form.

That has the address on it.
