I am the process on filling out the aadsas application and I ran into to a question that I am not sure what information to provide. For question A7 that asks about community service activities, am I suppose to provide a phone or some sort of contact information in case they want to check or am I suppose to provide that information on the secondary application. Also for question A9 where they ask about experience in a dental office, are they looking for work experience or is that where I can provide my shadowing experience.

I would make sure you list the name of the community service organization as well as the location (town, state or country) but you don't have to list names and phone numbers. eg I was a member of AmeriCorps National Service in Kansas City, Missouri. or something like that

Question A9 is where you would describe your shadowing experience.
