Just thinking about this churns my guts. Please don't be harsh. Here goes nothing:

Have you ever been disqualified, suspended, dismissed, or otherwise subject to a disciplinary action at any college or university in connection with your academic performance?

I went to a school straight out of high school and was academically dismissed for poor grades. Reason? Didn't have any motivation (no, no partying or anything). 🙁

I started "fresh" at a community college in my hometown and then transferred to a 4-year. I just want to submit my transcripts from the CC and current 4-year and am FEARING having to submit anything from the first school. Wtf do I write here? Below the answer box it says:

Note: Answering "yes" will not automatically disqualify you for admission to a dental program. Many individuals learn from the difficult experiences and emerge stronger as a result. Full disclosure enables programs to more effectively evaluate this information within the context of your credentials.

Dental schools may request verification from institutions previously attended that you were not involved in disciplinary actions. Failure to provide complete and accurate information may jeopardize your application.


"...will not automatically disqualify you". I feel like a lot of schools will. Can I pretend like I didn't do any schooling those 1.5 years at the first school? How bad will that part of my life look even though I've spent the recent 3.5 years or so "fixing" it? Will schools thoroughly investigate me and find out I went to the first school on their own anyway should I choose not to disclose that information?
 
You need to be honest...If they accept you and find it out later with background checks, you will be screwed.
 
And the stomach churning continues.

I guess I've screwed myself over from the beginning. I hope schools don't overlook my current gpa and DAT scores because of it.
 
Your gpa will be an average of EVERY college course u have ever taken. You r gonna have to put a positive spin on your situation in your app. Something must have changed in u to make u turn things around. It can be done. Good luck!
 
Just write everything honestly and you will be fine.
