I just want to know what it is like out there for a MD-PhD student. Who gets first authorship? and is it a norm or exception to be an author on paper with others from other department or agency or university?

First author is generally given to the person who contributed the most amount of work for getting the manuscript ready. Most of the time, they write it as well. It is quite common to find authorship with others from other departments and/or university.
 
Like Dr.Z said.

I don't think MD/PhDs are treated any differently than regular grad students when it comes to authorship.
