About undergrad school, DAT, apps. "name"

Do my names in undergrad school paperworks such as transcript and recommendation, DAT, and aadsas application all have to match?
The thing is.. my real name doesn't have a middle name.. but I added when I entered college..
So.. for examples.. My name in college is Andrew Jeff Woodward.
my name under social security is Andrew Woodward.
What name should I use when filling out apps and DAT???
I am really concerned.. Please help!

Do I have to change my school name??

Will my middle name bother w/ other stuff when applying to dental schools or when taking DAT??
