how many choices of answer we have for written and practical part? 4 or 5?

To be honest I can't remember. I think it was 5. But typically the questions are difficult because you have to decide between two responses. It's usually fairly easy to eliminate the others. They definitely have given up the k-type questions though that used to make you say whether just A was true, just B, both, or neither.

Does that count as remembrances?

I thought it was 4, but more importantly, agree with yaah that generally the challenge is choosing between 2 of the options.
