Hi any opinion or advice will be appreciated.

I will be a sophomore this fall. I had an incident where I violated my school's academic integrity codes. The violation is not on my transcript but it is on file. I was guilty for unauthorized collaboration on a lab report for chem lab. My bio1,2, chem 1, 2 are also not up to par, however, I plan on retaking those courses.

What can I do to even be able to apply to med school?
Is med school even an option at this point?
And if it isn't an option, what other career pathways should I pursue or consider?

You are early enough in the process to recover, I believe. Maybe you need to decide within yourself if med route is desired route. If, after enough time has passed that a more distinct picture of your overall package emerges, you can then most appropriately decide between md, do, pa, etc...don't feel there is no hope at this point, however.

Should I write about it in my personal statement or in a supplemental ?

Most schools have a basic info section on their secondaries where they ask if you've had an IA and ask you to explain it further. You could talk about it there.

If you're still a sophmore (or junior) and still taking prerequisites, forget the IA for now and focus on getting As in your coursework. It's useless now to worry about what to say on your PS or supplemental if you don't get the grades to be a viable candidate when it is application time. Plus by then you'll have 2-3 years of (hopefully) IA free achievement and maybe even take a gap year to put more time between you and the IA. Focus on what you can control (grades, MCAT, volunteer experience), not that which is beyond your control like the past.
