Hi,
I am transferring programs and am in good standing with my current program. I used up all my 4 weeks of allotted vacation time prior to the decision to transfer (it was sort of last minute) due to vacation and later emergencies. In hindsight, should have used FMTLA. I notice my GME office offers Academic leave (this is typically for people taking Step 3). I need 3-4 days to go start my next program on July 1st since its on the other side of the country and I have orientation a few days before that as well. Does anyone have any advice or ways I can go about this?

If you don't need a full year of credit for your new program, you can simply resign. You'll lose your salary for the time between programs, but that's probably not a big deal. If your leaving leaves the program short and causes problems, then you can expect that any job reference in the future may include this. So be careful. but you can certainly ask.

I'm transferring within the same specialty and my accepting program is taking me as a PGY-2. This was outside of the match and very late in the year. Also, I noticed I never took any of the 10 sick days we were allowed and did not take any time off for posters and such. I think my accepting program would be okay with me coming later on but I would have a whole separate orientation etc than the interns and it might set me back some.

I've been a good resident thus far and I don't think it's a big deal for the program I'm leaving to accommodate my request. It won't leave the program short to the extent that it will cause problems (I am on a rotation where I have a covering resident).

However, with all of this said, I do need one full year of credit to get my licensure (which is required by end of PGY-2 year). The other, not so fun alternative, is to ask my new program to give me a week of vacation from July 1-7th if possible.
 
Well then, you've answered your own question. You can ask your program if you can use some combination of sick/academic time to finish out the year and still get credit. They say yes, and you're all set. They say no and you stay till the end.

You probably can't take vacation at your new program until after you complete orientation. But your new program could just start your contract in the second week of July, if they want. It depends -- if the GME office organizes an orientation for that first week of July (usually for fellowships and advanced matched residents) they will probably want you to be present for that.

"What do your old and new PD say?"

aPD has outlined pretty much every possible scenario. Now you just need to ask the actual participants in your situation. Because nothing any of us says will matter.

When I talked to my new PD and PC they stated that they would find a way to work with me but would love it if I could come the last week of June. However, per my current PD, we are strapped for coverage and I need to stay until 06/30. However, my new program would need to find a way to accommodate my orientation, etc.

Sounds like you are stuck until 6/30
