Academic Private Practices?

Does anyone know of many 'academic private practices' out there? It seems like the only jobs I see out there fall in one of two extremes: (1) academic center associated with medical center (nice for teaching and research but inefficient in terms of patient care and surgery) or (2) private practice with emphasis on private (nice for efficiency, but seems like they just care about seeing/operating on 80 patients a day).

Ideally, I would like the efficiencies (read: minimal bureaucracy) of private practice but with the good-will and research connections of academics.

Do places like this actually exist?

In larger cities with academic programs, there are private-practice employed affiliate faculty who do at least clinical studies in their private practices (bench research, not so much). Some work under grants from public and private sources. I don't think you will find a public listing, unless the physician has public funding as from the NEI.
