California is very competitive.
Applied to FM At UCSF with a 246 ( state med school from the East Coast rank 60ish) and was competing with seniors from standford, ucsf, ucla, uci, uc davis etc.
Didnt make it there... Obviously.

Anyway, radiology in general is not as competitive as it used to be.
 
West Coast can be hard to break into. That being said, you can probably match into radiology somewhere on the west coast, especially if you are willing to go to a community program. Academic programs are tough because there are relatively few true academic programs on the west coast. A lot depends on the rest of your application. 242 is about average for radiology applicants. Can't speak to neurosurgery in any detail, but it would likely be a serious stretch.
