Based on most examples I'm familiar with, that seems true. In Prokaryotes, repressors in their active state bind to the operator. Activators can bind within the promoter, but I believe there are also some instances just upstream of the promoter as well (not 100% certain on this though).
Yes, activators usually bind either directly to the promoter or to an enhancer in the near vicinity, whereas repressors normally bind directly to, or on a silencer near to, the operator. Obviously there are exceptions to this generalization.
