How important is it to contact all the places I've worked/volunteered at prior to listing them in the activities section? It's just been so long and I don't think they'll remember me. Also, I could not find my supervisor's info from the hospital I volunteered at and I tried to reach out to them but nobody answered. Do you guys think that it's okay to list the volunteering department contact info instead? Thanks
 
Absolutely!!! You are totally fine. This is what we all do. No need to tell everyone you are applying to medical school and listed them on an application, It's not the same as asking for a reference or LOR!

Yes. The person contacted only needs to verify the timeframe of the activity you've listed. They won't be asked about your work ethic.
