I am in the process of sending out my recommendation letters and fees to dental schools and I am having problems finding the address to some dental schools. I was wondering if someone could please point me in the right direction? Also, I found the address to the Arizona School of Dentistry which is:

Donna Sparks
Arizona School of Dentistry and Oral Health
Admissions Office
800 W. Jefferson
Kirksville, MO 63501

Just to clarify when I send out the letter, should I omit Donna Sparks and just write Arizona School of Dentistry and Oral Health or does the letter need to specify be addressed to Donna Sparks. Also, would I send to recommendation letters and fees to the same address.
