Does anyone know if the PASS evaluations are actual letter or is it more of an evaluation form?

When you enter the names of your evaluators in the PASS website, you'll include their email addresses. They'll be sent a link to a "survey" where they will rank different character traits and will have the ability to enter some comments about you.
