Hello, everyone,

I need an advice..if anyone was in a similar situation. I am PGY1 categorical internal medicine...and I matched for advanced PGY2 psychiatry for 2013. How should I inform my actual PD without interfering with graduating PGY1 in good standing? He will force me to resign now? Do you think that my resignation will interfere with my future license applications? Any advice is highly appreciated...Thank you!

Shouldn't you have told him already?

I mean, shouldn't you have told him when you started to even consider switching?

Tell him now so he can find a replacement. They're probably making next year's schedule with the assumption that you'll be continuing. Notifying them any later may disrupt weeks of work. He's not likely to boot you early if you're working hard and doing your job. Everyone understand a change of heart.

Best of luck. Let us know how it goes.

But my resignation will affect my license in another state, isn't that?

So, you're a categorical internal medicine resident, but you've "matched" into a PGY-2 spot in a psychiatry residency?

How does that work? Was this a match through the NRMP? I would find it hard to believe that you would be allowed to attempt to match with a secured residency.

Have you already signed a contract for your PGY-2 year in your medicine program?

Is the psychiatry program aware that you're in a categorical medicine program? Were they aware when you applied?

It sounds like you've got a number of issues you may need to clear up ASAP

I don't think you are resigning. You are just choosing not to continue next year. You are switching.
This biggest issue here (assuming the program you matched into knows that you are/were currently an IM resident), is that you need to tell your current program director ASAP. They may be mad or even just irritated, but it's very unlikely they would fire you with <2 months left in 1st year. They need someone/a body to do the work. It doesn't seem like it would be in their interest at all to fire you. Anyway, you need to tell them now because they have to make the schedule for next year, and find another resident to fill your spot.

If it's for 2013, then this is clearly an out-of-match agreement.

Presumably, but 2013 would be for his/her PGY-3 year.

OP - it's pretty weird that you didn't tell your PD that you were doing this, but it seems like you should be fine. Just don't sign a contract for the 2013-2014 academic year.

yeah id like to know as well, becuase this is my dilemma, I cant even begin looking for a new spot without giving up my current spot. and I could end up not getting another spot, which is my big fear, that I tell my PD i want to transfer to a different specialty and he replaces me and then I dont find a transfer spot and end up jobless (which for a year wouldnt be too bad but if I cant get another residency Im pretty screwed due to my debt. although if I didnt have the debt I would definitely walk away from medicine as a career for EVER...).
