Question 1: Which opportunity is better for clinical experience?
1. EKG Tech at a hospital (12 hrs per week/paid)
2. Medical Assistant at a Dermatology clinic (8 hrs per week/ unpaid for the first 180 hours)
Question 2: I also need shadowing hours, which opportunity from the two above can provide me with potential shadowing opportunity?

Could you do both? Combined it’s only 20 hours a week. You need stand alone shadowing not tied to a volunteer or paid position. You need 50 hours of shadowing and a good bit should be with a primary care doc.

I am studying for the MCAT with 4 upper division science classes and 16 hours of research per week, I don't think I will have time to study for the mcat if I go to work 20 hours a week 🙁

1) Get paid off the bat

For shadowing, you need to see what a doctor's day is like. So no.
 
If finances are not a concern, MA seems to be the good one and you save 4 hrs for MCAT prep. You can shadow Dermatologist at same clinic and may be they can help you find primary care physicians to shadow.

I'd go for the EKG tech position (personally I just think that it sounds more exciting). Maybe you could shadow the dermatologist on the weekends (if they have weekend hours)?

It also of course depends on your own schedule/priorities. If you can't fit both things in this semester, maybe ask for an extension and take the opportunity to do the other thing next semester.
