I know we all hate grafts on here. Were all big time wound healers (myself included).
I have a guy who has had a stasis ulcer medial mal open 1.5 years. 8x5x0.1ish cm. Ankle Fx gone wrong. MRSA. Hardware removal. No osteo. Not infected. Treated by ID.
Seen several wound docs before me. Ive been treating for 8ish months.
Referred to vascular. Venous ablations performed. Arterial OK.
Tried topical steroids, collagen, Silver alginate, etc, etc. All under compression dressing(s).
Nothing.
Used a wound vac and got a 100% granular wound bed.
STSG under vac and at week 1 looked like it would 100% incorporate. At week 2 it near completely failed. <10% took.
Tried some other local wound care after failed STSG. Not budging.
Nothing.
Nushield application with debridement in OR and at 5 days 90% of the wound is epithelial tissue.
Im a believer. My n = 1.
I put 4 of those grafts on. they are 2.5 x 2.5cm. Im sure my OR manager will be contacting me soon.
