I took a summer school class at a local state school (that was not my University) that I was intending to take the following semester at my actual university. Thinking it would never follow me, I took it easy, got a bad grade (C-) and now am pretty sure I have to report it on my application. Is this so?

Do I have to report this score? If so, what weight is given to this course? I hope not! I worked hard all four years and graduated with a 4.0 and was hoping to put that on my application.

You're supposed to report everything. You can't selectively report classes. You're application can be thrown out if there is ANY misrepresentation. That being said, there is some debate as to how likely it is this sort of thing is found out.
 
Report it on your application. If you otherwise have a 4.0, it won't do significant damage to your GPA and adcoms will see that it was a one-time problem. You'd be in huge trouble if you failed to report it and med schools found out. It's up to you to determine whether or not you want to take that risk. It's probably not all that likely that a med school would find out if you lied, but I wouldn't want to take the chance if I were you.

you have to report all classes taken at any college up to the point of submitting your application.

eventually you will have to report all coursework to matriculate into any medical school...they require a transcript showing proof of completed courses and degree plan. But if you send your application in prior to finishing the class I might wait to update them with a transcript (bearing a mediocre grade) until after the adcoms vote on you. I havent updated schools about my summer grades (sent in my application early) and dont plan to until I get an acceptance offer or they ask me to.
