So lets say one gets a rejection from a prelim program, then later on gets an invite from the dermatology department of the same institution...is it out of line or futile to contact the medicine department to try to get a prelim interview?

This happened to me as well. Emailed the prelim saying I was interviewing with derm and they sent an invite within 24h - go for it!
