Suppose I rank an advanced program # 1 and a categorical program # 2 and no prelim program ranked me to match. Would I be bumped down to match at my categorical anesthesia program or would I match advanced and have to SOAP for a prelim ?

You'll have to go through the soap for a prelim spot. Ranking for your anesthesia residency is done first and if you've matched to an advance spot then it ranks your supplemental rank list.
