Try getting your non-clinical volunteering up. You should have 150 hours to avoid getting screened out at most schools (250 for high-metrics programs), and 40 hours is barely enough to get my attention. Ideally pick opportunities in food distribution, shelter volunteer, job/tax preparation, legal support, transportation services, or housing rehabilitation.
That said, you should focus on campuses that work closely with AHECs. You should include Belmont, Virginia Tech, Louisville, Kentucky, and maybe UAB. DO schools should also be in play if you aren't successful with MD's at first. Check for three-year primary care tracks.
