I am a Canadian citizen applying to US medical schools.

I applied to 20 schools about 18 days ago but I have only received 10 or so secondary applications. Do I need to call the schools that have not got in touch with me? Will I receive a rejection letter from the schools if I am declined pre-secondary applications?

Thanks for any help.

18 days is not very long at all, especially since you applied late in the cycle when the admissions offices are very busy. They will send you rejection letters if they decide not to send you a secondary.

I'd fill out the 10 secondaries and wait until 30 days have gone by before even calling or e-mailing to ask about a secondary. It won't affect your chances, but it's a bit more courteous to give them a fighting chance to even look at your application. If they downloaded it a week after your AMCAS was complete, and their admissions previewers took a week to look at it, and then another week to meet and decide what to do with it, and a week to send out a notice, that would be pretty standard.

If you must do something, you can get on AMCAS and do a status check to see if those schools downloaded your application.
