the all-elusive "corneal wedge"? i've read out this, heard about this and think it would be tremendously useful -- just can't get it in practice... narrow beam, oblique angle, ... never happens... any advice?

Practice, practice, practice. That is probably THE most useful thing to know in gonioscopy, esp helpful when there is no pigment anywhere in the angle. Try it on younger patients in their 20's and 30's as their corneas are very clear. Gonio everybody, eventually you will get. I may have been in my fellowship until I was completely comfortable with it. Good luck.
