Anyone help me word my three institutional actions?

I was wondering if anyone could help me word my x3 institutional actions that all occurred my fall quarter freshman year back in 2008. Basically had x2 noise complaints and we were found drinking in my dorm room october and november. The third IA happened in december and it was alleged I was smoking since my eyes were droopy and there was the smell of marijuana coming from my dorm even though I didn't smoke. Didn't fight it at the time because didn't know I would be trying to go to medical school now and I didn't know it would be a problem later on in my life. Any help would be appreciated. Thanks!
