Anyone here ever work in a transitional clinic?

Transitional as in moving from the inpatient to the outpatient setting. A clinic where kids who can't get timely appointments can be seen on a temporary basis to bridge them. I know the psych dept at UT San Antonio has a Transitional Care Clinic and they've published a few papers based on their process. Does anyone here have first-hand experience? What worked in terms of improving engagement (ie appointment attendance) and then making sure they were smoothly transferred to a long-term provider? What could a single provider with minimal support staff expect to get done alone?
 
If there is a need for transitional clinic, then this sounds like a disaster. The transitional clinic staff will default to become THE follow up. Why would the parents keep seeking a local provider when they have you? Why would they want to make the transition to another person when they have already told you everything? Let's say this clinic is hard nosed and will cut people off after 4 weeks, 6, 8, whatever time period it states it will transition for, now you are the last person to have written prescriptions for this patient when they drop off and decompensate.

This really sounds like a bad solution to an already difficult problem. I'd pass...

Transition will likely be better served by the Pediatrician - who will continue to say "I'm not the person to manage this... you need a C&A"
