A real pain in the ass if you don't in many states. Don't think its impossible in any, anymore at least. Does exclude you from working in for the federal government (VA hospitals, fed prisons/ fed med center, military), and will exclude you from many quality post-doc fellowships.

It's actually the EPPP (the national licensing exam). Licensure varies from state to state; some require that you practice under supervision for roughly 2000 hours after graduation (one year full-time) in addition to passing the EPPP and possibly taking a jurisprudence exam in order to be eligible, while a handful don't require the post-doc year.

"Postdoctoral fellowship" is a fairly general term. There are formal postdocs which represent one- or two-year (generally) formal training programs either in research or areas of specialized practice (e.g., neuropsychology, forensic psychology, severe mental illness, trauma, etc.). There are also informal postdocs, which basically is just a year of supervised practice somewhere under a licensed psychologist.
 
There are only a handful of states where you can literally be licensed right after your finish the postdoctoral internship. Even those that dont require some clinical hours and supervision post-doctorally, it often hard to really get the license in hand sooner than 6 months after finishing the internship. Most states still require post-doc hours and supervision before you can take EPPP and be licensed.
