Application Fee and Supplemental Info Format

For some schools, they request the application fee to be accompanied by additional info (your name, DENTPIN, AADSAS ID #, etc) are you guys going to just type that up and stick in in the envelope with your check? or address it to them and write a letter, including that information? I'm not sure what they are looking for.

For example, Maryland states:
The application fee should be accompanied by the applicant's full name, DENTPIN, current mailing address and telephone number.

What would you do in that situation?

I sent all my checks with a piece of paper (not letter) with my name, aadsas ID, dentpin, and date of birth
