Some schools require me to send in a picture along with the app fee. I did a search on the forum with no luck with this question.

Should the picture i send in be formal? Like suit a tie in a 2x2? or should it be a business casual type of pic?

Now to get this stupid pimple off my cheek...
 
Try searching again w/ "photo" and you should find plenty of comments on this. In a nutshell, it's supposed to resemble a passport photo, so make it 2"x2" with just your head and shoulders. Wear a collared shirt and smile.

I just sent in old prom pictures from 7 years ago.

I think as long as it's not a picture of you hitting a keg or anything you should be ok. Try and make it professional, this is professional school. Good luck.

On a serious note, yea just put on a nice shirt, collared shirt or anything nice and just smile. I got mine done at CVS. Was like $9 for 6 photos or something.
 
I let my wife take the picture with digital camera. Wore white shirt with tie and little smile to show that I have clean teeth. 😀 Got it developed it at Walgreens. 4 dollars for like 20 pictures at self-service machine😎
 
a word of warning...

while just about anything so long as you aren't half naked and pounding alcohol while holding a spliff will not affect whether or not you get accepted, most dental schools will use the pictures in some way, so even if it's not gonna hurt your app, you might want to consider an ugly passport photo or a prom picture from 1996 might end up hanging on a wall in the dental school for everyone to see for a year...

that's right, we've got 143 pictures on the wall on teh 2nd floor near the cafeteria, and some are wonderful and a lot are just horrible. I know all of those people and to look at some of those photos...some of those people look close to death (passport pics are some of the worst)

and if you don't think people look...just ask the other 800 people in the building if they've seen the pictures...they all saw them before we even got to the school
 
I took the picture myself and put it in photoshop... cropped it to be 2x2 and sent it in. I got an interview to Minnesota with it!
