Hello- I am currently a fourth year medical student applying for residency. I am planning on shooting for derm, but am a bit confused as to the process. Do we NOT have the ability as a 4th year to apply to ANY derm programs? I called one progrm and she said not until the Intern year- But aren't there some programs that accept you before and then you do the transitional year? I'm confused, because that would mean I don't apply for a residency this year, just a transitional spot?? Scary.
Any help is appreciated.

You apply for an advanced residency position (dermatology) and an intern year (med intern, trans, surg, peds intern, EM intern, OB/GYN intern...whatever you want) at the same time. You will interview for both spots this interview season (your intern year interviews will probably happen before your dermatology interviews for the most part).

On match day, you will hope that you match into an intern year program (to go to upon med school graduation) and an advanced program (dermatology) that you will begin directly after your intern year.

This is typically how it goes. Since this is the typical progression, it is not smart to only apply for an intern year and then apply during that intern year, since most of the advanced spots will be taken by those who applied for both simultaneously while in 4th year of med school. Additionally, it is not easy to find time to interview at programs during your intern year.

I have heard of some DO programs only taking PGY1's but I believe they are far and few in between
