Application question - work/activities - contact info

So I'm filling out some of my application even though I won't submit it for a year.
I am filling in the past work/activities section.
I worked in a nursing home for a year, about 5 years ago.
Shortly after I quit, so did my director or nursing and administrator.
I don't know what to put down as contact information. Should I put my former boss's name down and write in parenthesis (resigned) and continue to put the phone number of the facility anyway?

Any other thoughts?

Although a short experience, it is (one of) my most meaningful with a lot of hours, so I really don't want to leave it out.

You don't need a contact who will give you a reference, just someone who can confirm your dates of employment and hours worked. So use the nursing home's employment/human resources office as it will have the necessary records.
 
I thought that the information only saved if you submitted the application. It is fine to look at the application, but it is probably easier to type it up in word or excel.
