Under the evalutors section of the AADSAS application, where it is asking for the Title First Name Last Name of the evalutor. What are they looking for as title, Dr., Mr. or Mrs or something like Professor of Biochemistry. Also, if my university is going to send out the letter of recommendations to each dental school do I even need to fill this section out.

They are looking for something like Dr., not Mr. or Mrs., unless they're not doctors, which at that point you should enter either Mr. or Mrs.
