Anyone that has applied to FAMU's pharmacy school know how to send the Letters of recommendation and the "evaluation forms" to referrers? Do I print out the evaluation forms and fill them in?
Yes. What I did was print out the pages and filled in pg 2/4 because your signature is required THEN gave it to whoever and they must return it along with the letter of recommendation in a sealed envelope. I hope this was helpful!
