Applying to two different specialties and 4th year rotations

If someone is planning to apply to 2 specialties (eg. specialty A with B as back up) and that person schedules the first 3 rotations for specialty A and none for specialty B, wouldn't the PD be able to look at the transcript and catch on pretty quick?

Or do the PD's not get to see 4th year transcripts at all?
