Are 4th year grades automatically updated/transmitted to programs?

A friend of mine got an unexpected very poor [but passing] grade on a rotation that came after his Transcript was submitted to ERAS (on 9/15, per the ERAS site). Does he have to worry that this grade will be added to his uploaded transcript and transmitted to programs now or in the future?

Thank you so much for your responses ahead of time!

It probably depends on the school's policies on reuploading your new and updated transcript to ERAS. Certainly my school doesn't do any such thing unless you ask, and even then they probably won't. Once they got your application, that's probably all they're gonna see unless you go out of your way to show them more.

Programs won't see it unless the school uploads a new transcript. I doubt most schools do that routinely--I had to ask my school to upload a new transcript when the grade from an away rotation (in the specialty I'm applying to) came in a couple of weeks after ERAS went live.

No. By the time another transcript is uploaded or sent to the residency program he will have matched and signed his contract.

Somewhat related question: if I wanted to update my transcript on ERAS at this point (got a few 4th year grades back since ERAS including sub-I honors), will programs actually download them again from ERAS and have that factor in their rankings come January?

Yes, and maybe.

If you upload another transcript (which your dean's office must do, not you), then I will get it the next time I contact the ERAS postoffice.

Whether I will read it, or make any changes based upon what is on it, is anyone's guess. It certainly can't hurt, if your 4th year grades are good.
