27 week triplets for section last night....took arixtra 24 hours earlier, OB doesn't want to wait). Elimination half-life is 17-21 hours. When would you ladies and gents do a spinal, 3 half lives? Pent/ket/sux/tube for me.

The ASRA guidelines are quite clear on this. Or, to put it more bluntly, she's f*cked. It's a GA, unless you're ready to enroll her in a clinical trial...

Until further clinical experience is available, performance of neuraxial techniques should occur under conditions utilized in clinical trials (single needle pass, atraumatic needle placement, avoidance of indwelling neuraxial catheters). If this is not feasible, an alternate method of prophylaxis should be considered.

What she should be asking her OB (or other physicians) is who was the knucklehead who prescribed this in a pregnant woman, and a multiparous to boot.

There are more than just a few knuckleheads out there and they seem to be breeding like rabbits. Those of you in training have no idea.😱 I see pts all the time going through some of the craziest "medical" treatments b/c their doctors say they need it. Usually it is some wacked out FP doc trying to make some money, not in this case.
I had a pt come to me for a cervical fusion who received an "infusion" to help her recover from anesthesia. Some quack FP in town told her that the anesthesia for this surgery would keep her down for a week or more if she didn't let him give her this "infusion". When I asked her what it was she said that she didn't know. I then asked her why she would let somebody give her something like this if she didn't know what it was. She then started to cry and I knew I was deep in the ****. I called this quack to ask what it was he was giving but he wouldn't speak with me. Go figure.
