Does anyone have any good articles, links, or suggestions on how to substitute diltiazem products? They are really confusing, and just looking at facts and comparisons doesn't seem to help much.

The question on my mind is how do we identify patients who are eligible for IV to PO conversion of a cardizem drip. Then, how do we calculate the appropriate PO dose, and when do we start it.
