At what point are you committed to a school?

As I’m aware, if you get accepted off the waitlist at another program you can leave your seat and join that other program. But at what point is it not possible for you to leave your program to enter another off the waitlist? Once classes start? Orientation?

Are the medical schools that are scrambling to look for someone to fill a seat look at their applicants to see who is or isn’t accepted at a program already and determine whether it’s worth giving an acceptance to someone who is already accepted somewhere else and will start school shortly?
