Hi all,

For those of you who are PGY-IIIs or IVs, or know of those that are, I was wondering if you could describe the average caseload that an outpatient resident carries. Therapy vs. med mgmt would also be helpful. Also, I'm interested in the amount of supervision you receive in your respective clinics compared to patients seen.

No love? What about those of you who finished your residency outpatient work. Could you describe about how many patients you saw per week, your total caseload, supervision hours, or other clinical duties?

Oh... now you're resorting to us has-beens! From what I remember of PGY-3, I was expected to spend 20 hours/week on outpatient care. This included a total of 8 hours of: supervision (2 hours psychotherapy, 1 hour psychopharm), outpatient specific didactics, and administrative time (calling insurances, etc.). So that leaves 12 of "face-time" with patients. I think it broke down like this:

2 hours of group psychotherapy
6 hours of individual psychotherapy (all weekly psychotherapy cases, so caseload of 6)
2 hours of psychopharm clinic (4 x 30 minute slots, total caseload ~30)
2 hours of urgent care clinic

For PGY-4 they dropped my urgent care hours completely and decreased my outpatient caseload to 5 to allow time for chief resident stuff.
 
For me PGY-3 was:

Psychopharm clinic 1hr/wk (3x20min) - caseload 25-30
Psychopharm intake clinic 1.5 hr/wk

Psychotherapy intake clinic 1.5hr every 2 weeks

3-4 2x/wk psychodynamic therapy cases (often with meds)
7-10 weekly therapy cases (usually 10-12 cases because people would only come q2wks)

8 hrs of supervision per week - 3 psychodynamic supervisors, 1 psychopharm supervisor, 1 meds & therapy supervisosr, 1 CBT supervisor, 1 IPT or group therapy supervisor, 1 weekly intake supervision

Psych ER 1 full day per week

PGY-4 dropped to 3 supervisors (and one of them was PRN), no intakes, gradual purging of psychopharm case load and purging of therapy cases.

For psychopharm pts, the front desk would automatically schedule an interpreter. We would only do psychotherapy in languages in which we were fluent ourselves.
